The Core ISEF Forms Every Student Needs

Four forms apply to every single ISEF project, no exceptions.

Form 1: Checklist for Adult Sponsor. Your adult sponsor, usually a teacher or mentor, confirms they've reviewed your project and identified which additional forms apply. This is a routing document more than a research document, but it has to be right, since it determines everything else in your packet.

Form 1A: Student Checklist. You complete this yourself, walking through the same questions your adult sponsor answered, to confirm you both agree on which forms your project triggers.

Form 1B: Approval Form. This is the form that matters most. It has to be signed by you, your adult sponsor, and a Qualified Scientist if your project requires one, and it has to happen before you touch your experiment. Your isef research plan gets attached directly to Form 1B: a written narrative covering your research question, the prior work you reviewed, your methodology, and any safety or ethical considerations specific to your project.

Form 1C: Regulated Research Institutional/Industrial Setting Form. Required only if you conducted research at a university, industry lab, or any institution outside your home or school. It documents the institution's oversight of your work there.

Human Subjects Research Forms

Common Mistakes on ISEF Forms
